Admission Criteria :
|
Individuals 18 years and up with primary diagnosis of intellectual disability. Eligibility must be confirmed by DSO. Registration form including Plan of Care & Medical/Health Information form must be completed prior to scheduling a pre-placement visit.

Booking are generally based on first come, first serve basis. families may book up to 24 days per calendar year. Bookings may be done up to three months in advance.
Wellesworth Respite Program is partially wheelchair accessible (washroom, bedroom, living area).
