Family Information Package - A "starter guide" of things to consider when interviewing and contracting with an independent support worker through the CHAP Program
Some Things To Know About Me - A written profile template that can be completed and shared with an independent service provider
Blank Reference Form - If you would like to do additional reference checks. The CHAP Program does check three references for each independent support worker and these can be forwarded to you with the worker's permission.
