Admission Criteria :
New Visions Toronto’s Respite Program serves adults who are medically fragile, physically and/or developmentally delayed. Individuals must reside in the City of Toronto. Families who want to access its respite program will go through the screening process prior to involvement:
1) Phone intake
2) Home Visit
3) Completion of application and information package
4) Tour of respite location
5) Short visit to meet residents and staff
5) Confirmation of dates for respite visit
Activities :
Provides 24-hour pre-scheduled respite service for individuals who may have complex care requirements. Every effort will be made to provide individualized supports according to each individual’s needs. Proximity to many downtown attractions makes these locations a desirable one within the city.
