Family Registration

If you have previously registered with respiteservices.com, and have a received a username and password, please login to access your registration information and family profile.

If you are interested in accessing information on potential Direct Support Providers in Ottawa, and do not have a username and password, please complete the family registration form.

Your completed registration will be received by the Worker Bank Coordinator and you will be contacted within 2 weeks.

These highly recommended tools will assist you to provide your Direct Support Provider with detailed information about your family member in order to provide the best possible care in your absence.

**NOTE: You must submit the Family Registration, Consent and Family Agreement and Release (If registering to the Worker Bank) in order for your application to be processed.
