General Information :	The Local Health Integration Network (LHIN) is the access to respite services at Long Term Care Homes, as well as in-home support for respite funded by the Ministry of Health and Long Term Care. Case Managers, as specially trained health professionals, assit people with assessing their capabilities and explore independent functioning. If professional in-home services are required, the Case Manager can provide assessments, determine eligibility and level of care needs, arrange services and ensure care coordination of a multi-disciplinary team as required. Home care services are available on a visiting basis in the home including nursing and personal support to provide respite/caregiver relief. The Local Health Integration Network (LHIN) determines eligibility, maintains a central registry and waiting lists, and authorizes admission to long-term care facilities in Perth and Huron counties and the South West region. This includes short-term accommodation for respite or supportive care stays.
