Program Title :	Special Services At Home

Program Cost :	No fee for service
Admission Criteria :	Infants, children and adults with a developmental disability. In addition, children up to age 18 with a physical disability. The disability must be documented by a physician or psychologist.
Activities :	This program provides assistance for families/individuals wanting to access the Special Services at Home Program of the Ministry of Community and Social Services (see Funding section). The program provides individualized in-home support and community based activities for children and adults with a developmental disability and children under 18 years with a physical disability. Its focus is on "Personal Development and Growth" and/or "Respite Support" as determined by the family or individual. Family Services Perth-Huron assists with the application process as well as the administration of contracts once approved by the Ministry. This includes screening, matching and payment of service providers.
Comments :	Service providers are contracted, self-employed people who are carefully screened by our coordinators. In addition, they receive orientation and ongoing training and support.
