General Information :	Community Living Hamilton offers a range of options for individuals of all ages to spend some time away from their families. These options include both care provided in-home (ie:through Special Services at Home contracts) and out-of-home in either a staffed residential setting or an "Alternate Care" family home. Options can also include activity based programs (eg: weekly Adolescent Life Skills Groups; weekend or week long supported vacations or assistance with summer camps, etc.) which focus on involving individuals in activities while providing time for their family. The frequency of support offered varies according to individual and family identified needs and available resources.
Eligibility :	Referrals for all respite programs (with the exception of SSAH) must come from Contact Hamilton (905-570-8888). Individuals must have a primary diagnosis of a developmental disability and reside with their family in the City of Hamilton. Once a referral has been received from Contact, CLH staff will meet with the individual and their family to develop a respite plan.
