A Guide to Residential and Day Supports for Persons with Developmental Disabilities

What is the Collaborative Access Process?
•A mechanism that allows developmental service agencies to work together to find opportunities to meet requests for service.
•A streamlined way for individuals and their families to identify their residential and day support needs.
•A process comprised of two committees: The Focus Group and The Pressures & Priorities Committee. A representative from each developmental service agency in Frontenac, Lennox and Addington counties sits on the Pressures & Priorities Committee while the Focus Group is comprised of the executive directors from each of the developmental services agencies of Frontenac, Lennox and Addington Counties.

Who is eligible for Residential and Day Supports?
Any individual with a diagnosis of a Developmental Disability (i.e. full scale intellectual function at or below the second percentile) is eligible for developmental services.

When should I make a referral?
When you know your family member needs residential and/or day supports that are not readily available to you now.

How do I make a referral?
You may already be involved with one of our member agencies. To make a referral simply speak to your contact person or phone any one of the developmental service agencies.

What happens to your referral?
•The developmental service agency you contact will meet with you to discover what your needs are and will complete a comprehensive intake package. The developmental service agency completing the Pressures and Priorities package will assume the case management role.
•Your request for service will be heard at the monthly Pressures & Priorities meeting. The case manager will be responsible for keeping you informed.
•At the present time there are more requests for residential and day supports than placements available. For this reason individual needs are prioritized and your name may be placed on a waiting list.
•The Pressures & Priorities committee also works to identify available resources that may help to address some of your needs. Interim solutions may be explored to promote stability while awaiting a more permanent solution.
•When interim solutions are not enough, and your needs are critical, your case may be forwarded to the Focus Group for Case Resolution.

What is the Focus Group?
•The Focus Group is comprised of executive directors from each of the developmental service agencies in Frontenac, Lennox and Addington Counties.

•The Focus Group works together to identify and address community service gaps.
•Representatives from the Ministry of Community & Social Services and the Ministry of Children & Youth Services also attend Focus Group meetings.
•The Focus Group is responsible for the case resolution process.

What is Case Resolution?
The Focus Group reviews individual situations of an urgent need, referred by the Pressures & Priorities Committee, to identify any possible resources in response to this need. Case specific recommendations are identified and implemented within available resources. The Focus Group ensures that the appropriate Ministries are informed about service pressures within our community.
